Each annual membership plan allows patients to enjoy various forms of as well as a discount on all other procedures.
It is worth noting that these plans are not considered dental insurance and cannot be combined with other plans or discounts.

Healthy Gums Membership
Annual Cost (per patient):
- Adults – $349
- Children (under 13) – $299
This membership plan is designed for patients without dental insurance who have healthy gums. If periodontal disease is present, a Periodontal Membership Plan is required. Your smile deserves a healthy foundation and with this plan, we can help you build upon what you do at home to avoid dental issues before they arise.
Annual Membership Benefits (per patient):
- 2 professional cleanings (healthy gum patients only)
- 1 comprehensive exam
- 1 periodic exam (during recall visit) with X-rays as needed (bitewings or periapicals)
- 1 full mouth series of X-rays (FMX, as clinically indicated – panoramic not included)
- 15% discount on all other dental procedures (applies to our standard fee schedule)
- Priority scheduling for emergency visits

Membership FormPerio Maintenance Membership
Annual Cost (per patient):
- $599 (covers 3 periodontal maintenance visits + exams/X-rays)
This membership plan is designed for patients without dental insurance who have completed initial periodontal therapy (). Gum disease is a chronic condition that cannot be reversed past the point of gingivitis. Because of this, regular at-home and professional maintenance is required to keep gum tissue and teeth healthy. Paired with other desired procedures, this plan will keep your smile clean, protected, and beautiful.
Annual Membership Benefits (per patient):
- 3 periodontal maintenance visits (following completion of SRP)
- 1 periodic exam (during recall visit) with X-rays as needed (bitewings or periapicals)
- 1 full mouth series of X-rays (FMX, as clinically indicated – panoramic not included)
- 15% discount on all other dental procedures (applies to our standard fee schedule)
- Priority scheduling for emergency visits

Membership FormSRP Periodontal Disease Membership
Cost (per patient):
- Adults – $999
This membership plan is designed for patients without dental insurance who have been diagnosed with periodontal disease and require scaling and root planing (SRP) treatment. Once this phase has been completed and their condition is re-evaluated, patients are encouraged to then transition to a Periodontal Maintenance Membership Plan for ongoing care.
Membership Benefits (per patient):
- Comprehensive periodontal exam and diagnosis
- Full mouth series of X-rays (FMX, as clinically indicated – panoramic not included)
- 1 full round of scaling and root planing (SRP; usually completed in 2-4 quadrants depending on clinical needs)
- 1 periodontal re-evaluation exam (approximately 4-6 weeks after SRP)
- 15% discount on additional periodontal or restorative procedures recommended after SRP
- Priority scheduling for follow-up care
